Recipient Request Form

The Recipient Request Form helps us to understand your current needs, ensure funding is used fairly and effectively, and to keep your information up to date.

No matter how small or large your request is, submitting this form is the first step in the process. The board will not approve requests for reimbursement of an item that has already been purchased.

Important Notes

A new form is required for every individual request (even if you’re an existing recipient).

We may request additional documentation, quotes, or photos depending on your request.

Forms are reviewed by our board at our monthly meeting. You’ll receive a follow-up after your request has been evaluated.

Equipment Agreement

When Missouri KIDS provides adaptive equipment to a recipient, we ask that a simple Equipment Agreement be signed. This ensures the equipment is used responsibly and can continue to benefit others if it’s no longer needed.

What You’re Agreeing To:

  • Use with Care: The equipment is donated specifically for the recipient’s personal use and should not be sold or traded without written permission from Missouri KIDS.

  • Return If No Longer Needed: If the equipment is no longer helpful—due to changing needs or in the unfortunate event of the recipient’s passing—it must be returned to Missouri KIDS so it can be repurposed for another child.

  • Legal Acknowledgment: The agreement must be signed by the recipient or their legal guardian. If guardianship or power of attorney applies, documentation will be required.

Why It Matters:

This agreement helps Missouri KIDS make the most of every piece of donated equipment—ensuring that when a child outgrows a device or no longer needs it, it can bring independence and comfort to another young athlete in need.

Frequently Asked Questions

  • Yes. A new Recipient Request Form must be submitted for every individual need, even if you're already an approved recipient.

  • We encourage recipients to submit requests based on genuine need. We review requests quarterly and prioritize support based on funding availability and impact.

  • In most cases, the equipment belongs to the recipient. However, per our Equipment Agreement, if the item is no longer needed, it should be returned to Missouri KIDS to support another child.

  • No. Equipment cannot be sold or traded unless Missouri KIDS gives written permission.

  • Please contact us to discuss repair options or the need for a new request.

  • Yes. A recipient may apply for up to $3,000 per year, with a maximum of $12,000 during their lifetime. The maximum amount of scholarship money that can be awarded by the Board of Directors each year cannot exceed 3% of the balance of the Missouri KIDS investment account at the conclusion of the previous calendar year.
